Our standard rate is:
Intake session: $150.00.
Individual: $140 per 50 minute session
Couples/Families: $150.00/session (50-60 minutes)
We can offer a few reduced-rate slots for a limited amount of session. Please contact us with questions.
Sliding Fee Scale: (upon availability) For those who want to take advantage of the sliding-scale, feel free to contact me by phone or email if you would like to know what your fee would be prior to your first session.
Out-of Network Insurance: We do not currently take all insurances but you may still be able to receive some benefits, from your insurance provider. Many carriers will reimburse for out-of-network providers. You would need to call your insurance company to see if benefits exist, and if you would like to apply for reimbursement, I can provide you with a “superbill” which contains all the information you would need to send them, but you are responsible for all contact with them. You may also use an HSA (Health Savings Plan) as a form of payment.
In-Network Insurance: DG Counseling, LLC (December Goad), is in-network with some insurance providers/EAP’s and continuing to gain more. Please contact me regarding your current insurance and we can determine in-network or out-of-network benefits.
Forms of payment:
Cash or check are preferred and accepted at the beginning of each counseling session.
Visa, Discover, AE, or MasterCard are also accepted including cards associated with using your flex or health care saving account which offers tax savings too. You can choose to use your credit card on a regular basis or as a back up if you forget your therapy payment, etc.
Cancellation Policy:
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay $70.00 for the missed session.
Return checks fee:
$50.00 must be paid in cash plus the amount the check was written for. If the bad check is not resolved this agency will forward the check to the County Attorney’s Bad Check Division for assistance in collecting the restitution.
